Understanding the failure
Our team can provide answers in terms of anything from a simple component failure analysis through to a detailed Root Cause Analysis (RCA). Typical steps in an investigation might include:
- Site inspection soon after the event, anywhere in the world
- Interviewing operating personnel/other witnesses
- Gathering of information, photographs and records
- Laboratory-based forensic inspection of components
- Detailed mechanical, process or electrical simulation studies (e.g. Finite Element Analysis)
Mitigating the consequences
We can provide support to operators, insurers or loss adjusters to help minimise cost and
business interruption:
- Acting as a technical representative during remedial works
- Providing QA advice when dealing with suppliers
- Ensuring proposed repairs or replacements are fit for purpose
- Helping to understand the origins of damage and interruptions
Preventing recurrence
Avoiding a repeated loss is the true aim of any failure investigation. Many failures have a human factor root cause. We use Root Cause Analysis methodology to help:
- Identify the primary causes which led to the failure
- Develop strategies to reduce or eliminate these causes
- Produce practicable and economic recommendations
- Work with the client to implement these recommendations
- Provide training tailored to the client’s needs
